A33-year-oldwomanwhohadreceivedadiagnosisoftricuspid-valveendocarditiscausedbymethicillin-resistantStaphylococcusaureuswasevaluatedforreplacementofthetricuspidvalve.ShehadNewYorkHeartAssociationfunctionalclassIIsymptomsofheartfailure.Giantsystolicpulsations,knownasC-Vwaves,wereobservedonjugularvenousexamination(PanelAandVideo1).Transthoracicechocardiographyrevealednormalleftventricularsizeandsystolicfunction.Theleftventricularejectionfractionwasmorethan55%.Therightventriclewasmoderatelytoseverelydilated.Therightventricularsystolicfunctionwasmildlytomoderatelyreduced.Theleftatrialsizewasnormal,buttherightatriumwasseverelydilated.Amoderate-sizedvegetationonthetricuspidvalvewasseen,andtherightventricularsystolicpressurewaselevated,at30to40mmHg.Therewasseveretricuspidregurgitation(PanelBandVideo2).Duringsurgery,theposteriorleafletofthetricuspidvalvewasfoundtobeseverely
